AFFADVIT TO COLLECT PAST MILITARY PAY
State of Georgia, County of Lincoln: Before me Leonard Sims---anacting Justice of the Peace---in said county personally came WilliamGullatt, who being duly sworn, says he is the father of HendersonGullatt, deceased, late an orderly Sergeant in Co. "G," of the 15th Ga,Volunteers. He further declares that his said son died about the fourthof April 1862 and that he left neither wife or child surviving him. Andat the same time also appeared Jacob Zellars and made oath as aforesaidthat he knew the said Henderson Gullatt, his father, and the facts swornto by said William Gullatt are true and he is disinterested therein swornto and subscribed before me this 6th day of Dec., 1862.

State of Georgia, County of Lincoln: Before me James W. Murray, aJustice of the Inferior Court in and for the said County aforesaidWilliam Gullatt, the father of Henderson Gullatt, a soldier in Co. "G"15th Ga., Vols., saying he is the father of said Henderson Gullatt, thatthe said Henderson never married, has no widow or child and that hemakes this affidavit for the purpose of drawing the pay that may be duethe said Henderson Gullatt for his service in the Army of the ConfederateStates. Sworn and subscribed by me this 12th day of June 1862.

Individual Note: Served in Co. "G" 15th GA Infantry. Enlisted at Atlanta, GA, 7-16-1861,as a private. In March or April of 1863 he was hospitalized atGordonsville (?). On 6-15-1864 he was hospitalized at Richmond, VA. witha diagnosis of "Febris Int"(?). He was still in the service as late as1865. One record has him dying 8-30-1862 at Manassas, VA.
